    Instructions:
    1. With the assistance of the instructor in charge, fill in all appropriate information related to the directed study request.
    2. Obtain the signature of the departmental chair.
    3. Return the completed form to the Office of the Assistant Dean of Instruction.
    4. Upon final approval, you will be registered for the course in the designated term. Any changes made must be approved in accordance with regular academic regulations.
    5. If this is the only course that you will be registered for in any session, the full amount of the tuition and fees are due with the submission of the form.

    Note: The primary purpose of a "special topics" course or a directed study course is to pursue a subject of particular interest not already treated extensively in a regular course.
     
    Why do you need to take this directed study instead of a regular class during the Fall Semester, Spring Semester, or Summer Session?   
     I hereby request that I be permitted to earn credit by directed study. I have read the provisions of the Catalog, (Chapter 6) on this subject. I understand that I must petition the Academic Standards Committee if this course puts me over 18 hours this semester. I further understand that all charges will be due upon submission of this form to the Registrar's Office.
